Secret Santa questionnaire template
A few useful clues for the person shopping for you.
Keep it short
Send the same questions to everyone after they join. Give them permission to skip anything. Pass answers to their assigned giver, rather than putting personal details in a public group chat. A questionnaire gives ideas; it should not become a shopping order.

Ask only what you need
Skip sizes unless clothing is part of the plan. Do not collect addresses for an in-person exchange. If someone mails a gift, share an address directly with the giver and delete it when the exchange is done. Avoid asking for medical details; “no food gifts, please” is enough.
Stay inside the budget
An answer like “I love gardening” can guide a small, useful gift. Nobody needs to list expensive products. Let people include a couple of broad interests so the giver can choose something themselves.
Should everyone fill out every question?
No. Let participants skip questions and share only what they are comfortable giving to their assigned shopper.
